Grammar: Contrast and Concession — Atypical presentations

Contrast and Concession

  • In clinical practice, we often have to weigh conflicting patient data. Contrast and concession linkers allow us to highlight unexpected findings or compare different clinical presentations smoothly.
  • Linker
  • Grammar Structure
  • Clinical Example
  • although / even though
  • Followed by a clause (subject + verb)
  • Although the elderly patient had no chest pain, she was having a silent heart attack.
  • despite / in spite of
  • Followed by a noun, pronoun, or -ing form
  • Despite having normal ECG findings, the Boston resident ordered a troponin test.
  • whereas / while
  • Used to compare two contrasting facts directly
  • A typical presentation of MI involves crushing chest pain, whereas an atypical presentation may involve only mild epigastric discomfort.
  • nevertheless
  • Adverb used to start a new sentence (often after a semicolon)
  • The patient's initial symptoms were very vague; nevertheless, the clinical lead in London suspected an addisonian crisis.
  • much as
  • Used with verbs like like/respect/want to show strong concession
  • Much as I respect his diagnostic skills, I disagree with his decision to dismiss her symptoms.
  • Remember
  • Use 'although' and 'even though' with a full clause, but use 'despite' or 'in spite of' with a noun phrase or gerund.
  • Use 'whereas' and 'while' to balance two contrasting ideas or facts, often comparing two different patient cases.
  • Use 'nevertheless' as a transition word, usually at the beginning of a sentence, to introduce a surprising contrast.
  • Use 'much as' to express strong concession (often with verbs of feeling, wishing, or thinking) before a contrasting statement.
  • Examples
  • Even though the patient in Denver only had mild epigastric pain, Laura ordered an ECG immediately.
  • Despite the overcrowding in the Seattle hospital, the team maintained high standards of clinical governance.
  • The patient had experienced multiple falls; nevertheless, the senior resident initially dismissed the case as simple aging.
  • Common mistakes
  • Despite the patient was elderly, the London team did not check for delirium.
  • Although the patient was elderly, the London team did not check for delirium.
  • In spite of the clinical lead expected high standards, corridor care was still common.
  • In spite of the clinical lead's high standards, corridor care was still common.
