Incident Investigation Report: Unit 402 Collision

On October 14, at 03:15 AM, a serious multi-vehicle accident occurred on I-95 North near exit 78. Olivia, our senior account manager, had just finished an onboarding call for a highly valuable new account when the emergency alert arrived. Our driver, Lauren, was hauling a heavy load of auto parts. Although Lauren was worn out from a long week of driving, she remained fully alert and vigilant.

According to the official gate log from the shipper, Lauren had departed the warehouse on schedule. Thirty miles into her trip, she encountered a massive collision ahead. A passenger car had spun out on the wet asphalt and blocked two lanes. The highway was completely dark, and a dangerous traffic backup was rapidly forming behind the initial crash site.

Our safety team used the system to timestamp the exact sequence of events. The dashcam footage showed that before the collision occurred, Lauren had already noticed the hazard. She safely steered her tractor-trailer into the emergency lane. She quickly placed her warning triangles and worked to secure the scene, preventing further cars from crashing into the dark, disabled sedan.

While Lauren was securing the road, an SUV rear-ended her trailer at sixty miles per hour. Lauren was extremely shaken, but fortunately, she suffered no major injuries. She was taken to hospital for a standard checkup and was soon discharged. The state trooper arrived shortly after to coordinate emergency services and clear the closed lanes.

In trucking logistics, safety coordinators must write accurate reports. To do this, we use narrative tenses to explain timelines. The Past Simple describes completed actions (e.g., 'Lauren stopped'). The Past Continuous shows ongoing background situations (e.g., 'She was placing triangles'). The Past Perfect explains events that happened before another past action (e.g., 'The car had spun out').

Here is a quick guide to using these narrative tenses:
- Past Simple (Subject + Past Verb): 'The state trooper arrived at the scene.'
- Past Continuous (Subject + was/were + Verb-ing): 'Lauren was securing the road.'
- Past Perfect (Subject + had + Past Participle): 'The sedan had already spun out before Lauren arrived.'
Using these tenses correctly helps insurance companies establish who was at fault.

The safety department was thrilled to receive a moving letter from Sean, the driver of the sedan. He wrote: 'My family and I were terrified as we sat in our dark, damaged car on the highway. We were sure someone would hit us. Then, Lauren parked her massive truck behind us, shielding us from the oncoming traffic. She is our hero.'

Our client was initially stressed out about the delayed delivery, but they understood the heroic circumstances. Olivia successfully rescheduled the shipment. We are incredibly proud of Lauren's bravery. Her defensive driving saved lives, and our insurance carrier has confirmed that we are entirely not at fault. We have already submitted the complete police report and dashcam files.
