Austin Medical School Guide: Taking a History Step by Step
Welcome to the Austin Medical School guide for clinical students. When you walk into the waiting room, remember that first impressions matter a lot. Step one of taking a history is simple: sit down. Patients feel you listen more when you are at eye level, not standing over them.
After you sit down, start with a friendly greeting and ask about the chief complaint. While the patient describes the main symptom, like a sharp pain or a persistent headache, you must listen carefully. Do not interrupt too fast, because patients need time to explain their story in their own way.
Next, explore the details of the illness. Find out the exact onset and duration of the problem. Ask if the pain radiates anywhere or if there is a specific trigger that makes it worse. You can ask if the symptoms come and go throughout the day.
Then, check the medication list and ask about any known allergy to medicines. Although many people forget their exact doses, they usually remember the names. Do not forget to ask about family history and whether the patient is a smoker.
Finally, summarize what you heard so the patient knows you understand. If there is a power cut or an alarm goes off in the hospital, stay calm and finish your notes. Good communication helps every emergency doctor and radiologist later on.
